The Breast Center Navigator is a multi-faceted position providing comprehensive services to patients throughout the care continuum working in the role of patient advocate, educator and coordinator. Ensures patient comfort, satisfaction and well-being to optimize care and outcomes. Responds to patient questions, explains prescribed treatment plan and coordinates care across the healthcare continuum. Serves as a point of contact for referring physicians, patients, and families/caregivers to provide resources and assistance with accessing clinical and supportive care services.
Essential Duties
- Promotes a positive patient experience by following required policies and procedures. Treats each patient as their most important priority.
- Functions autonomously and in collaboration with other members of the AMI team as a patient advocate, educator, and coordinator.
- Provides assessment, care planning, education, coordination and consultation support to patients, families/caregivers, and clinical staff as the client transitions across the continuum of care. Interactions may be in person, by telephone or email.
- Provides direction and emotional support to the patient/family throughout the continuum of care. Educates patients and families/caregivers on diagnosis, prescribed treatment options, and treatment side effects and how to manage.
- Provides clinical resources, i.e., where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information.
- Answers questions as appropriate and serves as liaison to the physician for additional questions and concerns.
- Addresses tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, etc. Refers patients to the appropriate contact, such as asocial worker.
- Supports the timely scheduling of appointments, diagnostic testing, and procedures.
- Ensures any health information provided is accessible, i.e., securing documents printed in the patient’s own language or arranging for certified medical interpretation, in-person, or telephone/video conference.
- Assists with rooming, including vital signs, patient health history, and preparing patient for examination/procedures.
- Facilitates flow of patients in accordance with appointment time to avoid delays for the provider and undue waiting for patients, including keeping exam rooms stocked, clean and ready for use.
- Completes patient chart in a thorough and timely manner by recording health information, examination, treatment, and test results.
- Assists in Breast Tumor Board preparation and attendance.
- Meets patients on-site and remotely on the day of surgery.
- Assists with data collection and evaluation for quality assurance, performance improvement and regulatory reporting to ensure quality of services provided to patients.
- Assists in process design and educational activities, program execution and outcomes.
- Performs other duties and special projects as assigned by management.

- Computer proficient with Microsoft Office applications (Word, Excel, Outlook)

Certifications & Licensure
- RN License in good standing as recorded on the Nebraska State Registry required
- Current Basic Life Support Certification (BLS)
Education & Experience
- Successful completion of a Registered Nurse course of study
- Two years nursing experience
- Prior experience in oncology nursing preferred
- Specific training for navigation or coordination of care for the breast patient desired
